The final report linked a fatal predawn collision with a hazard already visible in several earlier incidents.
A tree across the tracks
On October 14, 2024, a southbound River Line train struck a fallen tree near Florence before dawn. A branch penetrated the forward windshield, fatally injuring the operator. Twenty-three passengers were taken to a hospital with minor injuries.
The NTSB attributed the collision to low light that prevented the operator from seeing the obstruction soon enough to stop. The completed investigation also identified four earlier collisions with fallen trees. Those incidents had been minor, but they demonstrated an existing hazard.
From individual trees to a safety program
The board emphasized incorporating measurable hazards into system safety programs. NJ Transit and Alstom completed a corrective plan to remove tree hazards, with state oversight monitoring continuing efforts.
The report does not say that every earlier tree strike had the same cause or that future risk had disappeared. Its contribution is the connection between apparently small events and the need to recognize a pattern before a fatal collision.
